To Place Your Order By Mail or Fax:

  2. Completely fill in all information in ledgible print.
  3. Calculate Order Sub-Total.
  4. OK Res. call for tax
  5. Calculate Shipping Rate or call for rate.
  6. Add up Total.
  7. Sign the Order Form.

  8. Send Cashier's Check, Money Order (Made payable to Starlight Entertainment), or fill out the Credit Card Information section.
